I was in the same boat. I was able to secure the bracket with a 1/4 x 3/4" Allen head bolt. Access is really difficult but the bracket on the inside of the door is actually a "U" shaped bracket and a hole can be drilled on the face of the door. I called Toyota and they said I was beyond the 5 year warrantly period. I am the 2d owner for my 2006 Sienna, initially purchased Fall 2005. I had it repaired at a body shop for $650. One body shop said they would not know until they took apart the door if they could repair or would need to replace the door.

Has this problem been resolved? I am having identical issues with a 2004 Sienna. I cleaned the throttle body which was pretty grungy. Did not help. I believe excess fuel is sitting in the engine on startup. If you wait long enough, the fuel will evaporate. This not a fix but try cracking the accelerator pedal a bit to get more air in the engine. I have a 2004 Sienna with 116K miles. The original radiator leaked and was replaced under warranty. While in for timing belt service I was truthfully informed that replacment radiator is starting to leak in minor way. The service rep used the word "weep". I'll be doing the third replacement myself. I have replaced many radiators in vehicles that I've owned, but never second time.
I feel for you and numerous others with the same problem, I have the problem too. I just encourage everyone to file a complaint with Toyota, this has been going on too long!
You'll find used Toyota's are way overpriced. If you want a much lower price either buy a Chrysler (Caravan or Grand Caravan) or GM (Uplander or Montana). I just bought a 2007 Uplander LT (SWB) with 55k miles for about $11k (taxes incl.). The dealership had a 2004 with 133k miles for about the same price and a 2005 with 88k miles for about $15k. Also, You can buy a remanufactured hinge instead of a brand new one. This might save you some bucks.....
I saw my mechanic fix it by loosing up the door from the body without disconnecting all the wire. Make sure the automatic closing mechanism switch is turned off. After loosing it from the body, you can take out the hinge and put a new one. It will require 2 people to fix this problem. My cost is $250.00 including parts. It took him 3 hours. Good luck....
yomama, Unfortunately you will have to remove the door to replace the middle hinge. I recommended replacing the upper hinge while your at it also. I have a technical service bulletin (TSB for the 2004 Sienna) that shows how to replace the hinge and motor assembly. My friend and I replaced both hinges upper and middle hinge in about 3-4 hours. A fellow posted earlier that he had fixed his with new steel rivets. I performed the same procedure on my 2004 (130,000 miles) using extra hard bolts after drilling out the spot welds. Use only high quality drill bits. Access is not easy with wrenches and drill but certainly do-able. My understanding is that any defective part that poses a safety risk is to be recalled by law. Toyota clearly downplayed the safety risk and got around the law. This reply is a little late, but my 2004 has the same problem-no speedo ,no odo, and check engne lite-they say it's the abs actuator assembly-something like 2 thousand ++ for the part. My sienna only has 26,000 miles. Needless to say, I'm pretty disappointed and will write a letter to Toyota. Maybe it's not such a rare problem?

TSB's are a bit of a strange "animal". The factory instruction to the dealer service personel is that the customer should NOT be informed of a TSB unless the customer first states a complaint which is covered by a TSB. It seems all the Japanese vans (Sienna, Odyssey, MPV, Quest) all have some type of trnasmission issues That's a myth. Odys trannys have been fine since about 2004 or so. I doubt you're shopping for something 2003 or older. Siennas never had trans problems. at least Korean cars have 10yrs warranty The powertrain warranty would not cover the door cable. A 2004 Kia's B2B warranty would have expired in 2008 or 2009, depending upon when you bought it. You may want to consider an extended warranty, just make sure it covers your door.

I have paid $2,000 for brake service since buying my 2002 Sienna van. Now the shop wants to replace the front struts and shocks, saying that they are weakened due to all the weight of the van during stopping transferring to the front brakes/axle. It's fairly easy, if not expensive. Just go to your Toyota dealer and tell them which model and year you have and they can look it up. Most probably they will have to order it, for about $200. I am having a similar issue with our 2005 Sienna. It started out with not blowing cold when I'd have a/c on..and I'd have to turn it off and then turn it back on to get the cold air to engage.
